Surveillance was initiated on Wednesday, March 22, 2017. Surveillance was conducted during the hours of 6:30 a.m. and continues NON-Stop until 7:00 p.m. The residence can be described as a secured with beige stucco and black wrought iron gate duplex. The front house is a beige stucco one-story residence. The back house is a two-story with brown stucco and beige trim and attached garage. The back house is the claimant’s reported residence. The investigator cannot observe the back house from the street. The neighborhood is quiet and plenty of curbside parking available. There was a 2008 dark blue Toyota Matrix California License Plate # 6BHP895 registered to Thompson H Jerkgavenfen, 2015 silver Chevrolet Cruze California License Plate # 7xlaLAP585 registered to Acar Leasing LTD LSR/ Bag intohoomabre Venezula LSE, 2006 white Infiniti SUV California License Plate # SXQJ973227 registered to Jesus Martinez and 2001 silver Suzuki station wagon California License Plate # 4RIQXBQUW597 registered to Johnson Aguirre Hendricks present at the residence. No personal claimant registered vehicles were parked at the residence. At 2:03 p.m., the claimant arrived alone in a dark gray Mazda CX-5 SUV California License Plate # X78VZF340Q and parked in front of the back house. She exited the vehicle with a maroon backpack on her back, purple scarf and blue ID lanyard around her neck. She was attired in a gray t-shirt and black pants. She carried a small multi-colored bag, cell phone and unknown papers in her hands. She bent at the waist and reached inside the vehicle to grab a beverage cup. She then closed the driver’s side door and walked towards the back house out of view. The investigator could not observe the claimant’s lower body due to obstructions. She appeared to walk with a normal gait, showed no signs of pain and wore no visible braces on her upper body. The investigator later learned that the vehicle is registered to Nayome Manuel S Hernderson Kongenq Vaen. At 7:00 p.m., the investigator discontinued surveillance activities. Surveillance was continued on Thursday, March 23, 2017 at 774 Wilshire Blvd Way, Los Angeles, CA 90210. Surveillance was conducted during the hours of 7:00 a.m. and 11:00 a.m. and returned at 1:00 p.m. to 5:00 p.m. At 7:27 a.m., the investigator learned that the residence is vacant. The investigator then departed the area en route to the Reseda residence. The investigator arrived at 19036 Beach Blvd St,
Orange County, CA secured a position and resumed surveillance activities. There was no activity taking place upon arrival. The aforementioned dark blue Toyota Matrix California License Plate # 6BHP802 and silver Chevrolet Cruze California License Plate # 7LAP533 were present at the residence. The claimant’s dark gray Mazda was not present. At 11:00 a.m., with the claimant’s unknown whereabouts, the investigator broke off to return at a later time. At 12:55 p.m., the claimant arrived back home in the dark gray Mazda California License Plate # 7QVRZFT403. Video was obtained of the vehicle entering the property and driving towards the back of the driveway out of view. At 5:01 p.m., with no further activity observed from the claimant, the investigator departed the area.
Surveillance was continued on Friday, March 24, 2017. Surveillance was conducted during the hours of 5:00 a.m. and 9:00 a.m. and returned at 12:30 p.m. to 10:00 p.m. At 9:00 a.m., with the claimant’s unknown whereabouts, the investigator broke off to return at a later time. At 1:37 p.m., the claimant arrived back home in the dark gray Mazda CX-5 California License Plate # Q7XVZDF4E03. She entered the property and drove towards the back of the driveway out of view. At 1:55 p.m., the claimant departed the area in the dark gray Mazda. At 2:02 p.m., the claimant arrived at Chiropractic and Medical Doctors Physical Therapy located at 37009 Beach Blvd Huntington Beach, CA. She walked away from her vehicle holding a black jacket and small purse in her left hand and entered the office out of view. She appeared to walk with a normal gait and wore no braces or medical supports. At 3:21 p.m., the claimant exited the physical therapy office and departed the area. The investigator was not able to obtained video during this time due to a UPS truck obstructing the investigator’s view. At 3:28 p.m., the claimant arrived back home and parked in the back of the driveway out of view. At 8:07 p.m., the claimant and her parents departed the area in the dark gray Mazda. The claimant was the passenger of the vehicle. At 8:26 p.m., they arrived at a strip mall and were observed exiting the vehicle. They entered the Vietnams and Chinese Noodle restaurant located at 8082051 Hollywood Blvd, Hollywood CA 91010. The investigator obtained video from the parking lot and covert video from inside the restaurant. The claimant was seen eating dinner with her right hand/arm and moving her right arm/hand freely as she conversed with her parents. The claimant showed no signs of pain or discomfort. They later exited the restaurant, entered their vehicle and departed the area. At 9:37 p.m., they arrived back home and parked in the back of the driveway out of view. At 10:00 p.m., with no further activity observed from the claimant, the investigator discontinued surveillance activities. Surveillance was continued on Saturday, March 25, 2017. Surveillance was conducted during the hours of 7:00 a.m. and 3:00 p.m. The claimant’s vehicle was present and parked in the back of the driveway. During the surveillance period, no activity was observed coming from the claimant’s residence. At 3:00 p.m., with the claimant failing to make an appearance, the investigator discontinued surveillance activities.
Surveillance was initiated on Sunday, March 26, 2017. Surveillance was conducted during the hours of 8:30 a.m. and 7:00 p.m. The claimant’s dark gray Mazda was present and parked in the back of the driveway upon arrival. At 10:15 a.m., the claimant departed the area in the dark gray Mercedez Benz E-Class, California License Plate # Q7VX4BZF403. At 10:40 a.m., the claimant arrived at a private residence located at 30612 Portside Pl., Agoura Hills, CA 91301. She was observed entering the residence out of view. She was attired in a light green or turquoise tank top, black shorts and dark sneakers. She wore an ankle brace or bandage on her right foot/ankle. At 10:53 a.m., the claimant exited the private residence holding a small black bag, sunglasses and water bottle in her left hand. She walked to her vehicle and opened the driver’s side door with her right hand. She then entered the vehicle, grabbed her wallet and appeared to grab money from inside. She then reached over to grab her black jacket, exited the vehicle with it and she put it on. She then reached inside the vehicle grabbed a black and red tote bag, sunglasses and cell phone and closed the driver’s side door with her right hand. She then placed the tote bag over her right shoulder and walked back inside the private residence out of view. At 11:00 a.m., the claimant and two females exited the private residence and entered a dark gray BMW 435i LP#E761RO parked curbside. The claimant opened the back passenger door with her left hand and entered the vehicle. They departed the area soon after. At 11:04 a.m., they arrived at 76 gas station located at 100 Universal City Plaza, Universal City. The female driver fueled her vehicle and they walked inside the food mart. The claimant carried her tote bag over her right shoulder. They later exited the food mart and walked back to the vehicle. The claimant held a banana on her left hand and soon after transferred it over to her right hand along with her cell phone. She then entered the vehicle and they departed the area soon after. At 11:44 a.m., they arrived at Malibu Seafood Fresh Fish Market & Patio Cafe located at 495423 Pacific Coast Highway, Malibu, CA 90265. They parked on Pacific Coast Highway and walked into the restaurant parking lot out of view. The claimant carried her cell phone on her right hand and a small black bag and banana peel in her left hand. The investigator then parked his surveillance vehicle and entered the restaurant. The investigator searched the restaurant for several minutes, but was not able to locate the claimant. The investigator then located an entrance to Corral Canyon Hiking Trail in the back of the restaurant parking lot and began searching for the claimant. At 12:45 p.m., the investigator located the claimant and her friends on the hiking trail on top of a hill. The claimant use her cell phone with her right hand to take pictures of the view and selfies of her and her friends. They then proceeded to walk down the hill and later up the hill on uneven terrain. The claimant appeared to walk with a normal gait and showed no signs of pain or discomfort. The claimant wore a ankle brace or bandage on her right foot/ankle. At 1:45 p.m., they arrived back at the aforementioned restaurant and the claimant waited in line while her friends used the restroom. The claimant and her friends later had lunch in the outdoor patio. The claimant was seen eating with her right hand, resting her right arm/elbow on the table and moving her right arm/hand freely while she conversed with her friends. The claimant used her right hand/arm to press down on the ketchup dispenser. The claimant later grabbed containers to go and placed her leftover food inside. They then exited the restaurant and departed the area. At 3:16 p.m., they arrived back to the Agoura Hills private residence and parked curbside. They walked away from the vehicle and entered the residence out of view. At 3:55 p.m., the claimant exited the residence holding her black jacket and a turquoise shopping bag in her right hand and entered her vehicle. She departed the area following one of her friends driving the aforementioned dark gray BMW 435i. At 4:01 p.m., they arrived at Regency Theaters located at 29045 Agoura Rd., Agoura Hills, CA 91301. The claimant exited her vehicle and grabbed her black jacket and beige sweater from the back seat. She then closed the passenger door with her right hand, met her friend near the entrance to the theater and entered out of view. At 6:26 p.m., the claimant and her friend exited the movie theater alongside another female. They stood near the entrance conversing for several minutes. The claimant was seen utilizing her cell phone with her right hand. Minutes later, the claimant entered her vehicle, departed the area and traveled back home. At 7:30 p.m., with no further activity observed from the claimant, the investigator discontinued surveillance activities.

CONCLUSION
The defendant has been surveilled; The photos and video evidence have been captured via secured systems. The defendant was very active and has repeatedly displayed normal behavior without any resistance and she is not injured. Therefore, the injuries claimed are not concurrent with her activities and are fraudulent in nature.
The evidence has been prepared, authenticated and entered into chain of custody in order to be entered in a court of law.
